- UPSSSC PET 2023: Candidate must have appeared in PET 2023 and possess a valid score (Non-Zero/Non-Negative).
- Academic: Intermediate (10+2) passed from a recognized board.
- Technical: Successful completion of ANM (Auxiliary Nurse Midwife) training course (1.5 or 2 years) recognized by the Indian Nursing Council.
- Registration: Valid registration with the Uttar Pradesh Nurses and Midwives Council.
